A functional health information system (HIS) is critical for monitoring immunization coverage and providing routine data to inform immunization service delivery and planning. This study in Ethiopia implemented a household-based population survey linked with a facility survey to assess childhood immunization coverage, HIS performance at the facility level (including human resource capacity and system infrastructure), and the relationship between them.
